Swasthya Sathi Card is a government issued unique identity card in West Bengal which provides healthcare benefits to people of lower income class. The Swasthya Sathi card enables beneficiaries in getting cheaper healthcare and medicines. Like PMJAY, It provides free treatment of upto Rs.5 lakh for poor people even in private hospitals. The payment is made via smart card or Swasthya Sathi card and beneficiary doesn’t need to pay on their own.

Check Swasthya Sathi Card Status by URN Number
To check the status of Swasthya Sathi Card online using URN number, use following method –
- Go to https://swasthyasathi.gov.in/
- Go to MIS section in the left side of the website.
- Click on ‘Card Verification’ option to check Swasthya Sathi card status by URN number.
- Select West Bengal in state’s option.
- Choose your district.
- Enter your Swasthya Sathi Card URN number.
- Submit the form and your Swasthya Sathi Card status will be displayed there.
How to check Swasthya Sathi Card status by application number?

Check Swasthya Sathi Card Status by Aadhar Number
If you do not remember your URN number, you can also Check Swasthya Sathi Card Status using Aadhar number and mobile number. Use following step to know status of Swasthya Sathi Card by Aadhar –
- Go to https://swasthyasathi.gov.in/ website.
- Click on ‘Find Your Name’ link from menu.
- On next page, enter your registered mobile number.
- Select whose Swasthya Sathi Smart card it is.
- Submit the form and then open the dashboard.
- Select the name of state and district.
- In ‘Select By’ option, choose Aadhar Card.
- Enter the Aadhar Number to search your Swasthya Sathi Card.
- Submit and your smart card status will be shown there.
Check Swasthya Sathi Online Registration Status
If you have applied for the Swasthya Sathi card and you want to check the application status or registration status of your card, use following steps –
- Go to https://swasthyasathi.gov.in/
- From Menu, select ‘Apply Online’.
- Then, click on ‘Check your Online Application Status for Swasthya Sathi card.
- On next page, select your district.
- Select the search criteria from three options – Aadhar number, Registration number and URN.
- Enter the details as per search criteria selected.
- Submit and check your Swasthya Sathi Card application status online.

- Click on ‘Check Hospital Empanelment Status’ option.
- On next page, enter registered mobile Number.
- Click on Search button to check hospital empanelment status.

URN number is Unique Registration Number given to all beneficiaries who have applied for the Swasthya Sathi Card.
You can check your Swasthya sathi card status by mobile number too. But you’ll need additional information for the same, like document number submitted with application.
