• Fill the enclosed form and submit to district magistrate or commissioner of KMC, Kolkata Municipal Corporation.
• Every applicant needs to submit Prochesta application form individually only.

Following information is required to fill the Prochesta form. Prochesta Prokolpo applicants need to submit following documents and details –

• Name of applicant
• Father’s name
• Gender
• Date of birth
• Epic voter ID
• Digital Ration Card number
• Aadhaar card number

Contact details

• State (West Bengal)
• District, Assembly, Block and Municipal Corporation
• GP/Ward number, Area, House, Post Office and Police station.

Bank account details

• Bank Name
• Branch Name
• Account Number
• IFSC Code

Self declaration

Applicants need to declare that they are eligible for Prochesta application form and are not beneficiary of Samajik Suraksha Yojana, Social Pension Scheme, or any of benefit of social scheme by West Bengal State Government.

Prochesta Prokolpo Application Approval Process – wb.gov.in

• In rural areas, Prochesta Application shall be accepted and reviewed by BDO or Block Development Officers.
• In urban areas, same with be done by SDO.
• The prochesta scheme application form will then be sent to DM and the verification of applicant shall be done.
• After that, the scheme benefit will be sent to the beneficiaries.
